RELEASE AND WAIVER OF LIABILITY
This RELEASE AND WAIVER OF LIABILITY (this "Release") is executed as of May, 2026 by and between _______________________________ (the “Participant”) and Good News Christian Center, Inc. d/b/a Elevate Life Church CT, a Connecticut nonstock corporation, and its directors, officers, employees, volunteers, and agents (collectively, the "Organization"), each a “party” and collectively, the “parties”.
RECITALS
The Participant desires to engage in those activities sponsored by the Organization, which include, but are not limited to, those activities described in Exhibit A to this Release, which are incorporated herein and made a part hereof (the “Activities”). The Participant understands that he/she will receive no compensation or remuneration for the Participant’s participation in said Activities.
In exchange for being allowed to participate in the Activities and for other good and valuable consideration, the receipt and sufficiency of which the Participant acknowledges, the Participant hereby freely, voluntarily, and without duress executes this Release and agrees to the following terms:
- Compliance with Policies. The Participant acknowledges and agrees that he/she will comply with all applicable policies and procedures, training, and safety rules of the Organization, and will follow the Organization’s instructions while carrying out the Activities.
- Assumption of Risk. The Participant is aware and understands that the Activities may be inherently dangerous and may expose the Participant to a variety of both foreseen and unforeseen hazards and risks. The Participant acknowledges that he/she is voluntarily participating in the Activities and has considered those risks. The Participant hereby expressly and specifically assumes such risks, including any and all risk of injury, harm, or loss that the Participant may incur as a result of his/her participation in the Activities.
- Medical Treatment. The Participant hereby gives consent and authority to the Organization to obtain medical treatment on the Participant’s behalf if the Participant is injured or requires medical attention during his/her participation in the Activities. The Participant understands and agrees that he/she is solely responsible for all costs related to such medical treatment, medical transportation, and/or evacuation. The Participant and his/her personal representatives, assignees, insurers, heirs, executors, administrators, spouse and next of kin (collectively, the “Participant Parties”) hereby release, forever discharge, and hold harmless the Organization from any claim whatsoever in connection with such treatment or other medical services.
- Release and Waiver. The Participant and the Participant Parties hereby fully and forever release and discharge the Organization from, and expressly waive, any and all liability, claims, liabilities, allegations, demands, obligations, actions, suits, causes of action, debts, costs, expenses, and controversies of whatever kind or nature, either in law or in equity, that may arise from the Participant’s participation in the Activities. The Participant and Participant Parties agree not to make or bring any such claim or demand against the Organization, and fully and forever release and discharge the Organization from liability under such claims or demands.
THE PARTICIPANT UNDERSTANDS THAT THIS RELEASE DISCHARGES THE ORGANIZATION FROM ANY LIABILITY OR CLAIM THAT THE PARTICIPANT MAY HAVE AGAINST THE ORGANIZATION WITH RESPECT TO ANY BODILY INJURY, PERSONAL INJURY, ILLNESS, DEATH, PROPERTY DAMAGE, OR PROPERTY LOSS THAT MAY RESULT FROM THE ACTIVITIES, WHETHER CAUSED BY THE NEGLIGENCE OF THE ORGANIZATION OR OTHERWISE.
- Insurance. THE PARTICIPANT UNDERSTANDS THAT THE ORGANIZATION DOES NOT ASSUME ANY RESPONSIBILITY FOR OR OBLIGATION TO PROVIDE FINANCIAL ASSISTANCE OR OTHER ASSISTANCE, INCLUDING BUT NOT LIMITED TO MEDICAL, HEALTH, OR DISABILITY INSURANCE OF ANY NATURE IN THE EVENT OF THE PARTICIPANT’S INJURY, ILLNESS, OR DEATH, OR DAMAGE TO OR LOSS OF THE PARTICIPANT’S PROPERTY.
- Indemnification. The Participant and Participant Parties hereby agree to indemnify, defend, and hold harmless the Organization from any and all liability, losses, damages, judgments, or expenses, including attorneys’ fees, that it may incur or sustain as a result of the Participant’s negligence, recklessness, or willful misconduct in connection with the Participant’s participation in the Activities, arising out of any third-party claim.
- Photographic Release. The Participant understands and agrees that during the Activities, the Participant may be photographed and/or videotaped by the Organization for internal and/or promotional use. The Participant hereby grants and conveys to the Organization all right, title, and interest, including but not limited to, any royalties, proceeds, or other benefits, in any and all such photographs or recordings, and consents to the Organization’s use of the Participant’s name, image, likeness, and voice in perpetuity, in any medium or format, for any publicity without further compensation or permission.
- Entire Agreement. The Participant hereby agrees that this Release represents the full understanding between the Organization and the Participant and supersedes all other prior agreements, understandings, representations, and warranties, both written and oral, between the Organization and the Participant, with respect to the subject matter hereof. This Release is binding on and inures to the benefit of the Organization and the Participant and their respective heirs, executors, administrators, legal representatives, successors, and permitted assigns.
- Severability. If any term or provision of this Release shall be held to be invalid by any court of competent jurisdiction, that term or provision shall be deemed modified so as to be valid and enforceable to the full extent permitted. The invalidity of any such term or provision shall not otherwise affect the validity or enforceability of the remaining terms and provisions.
- Headings. Section headings are for convenience of reference only and shall not define, modify, expand, or limit any of the terms of this Release.
- Terms. Common nouns and pronouns shall be deemed to refer to the masculine, feminine, neuter, singular and plural, as the identity of the party or parties may in the context require.
- Governing Law. The Participant hereby agrees that this Release is intended to be as broad and inclusive as permitted, and that this Release shall be governed by and interpreted in accordance with the laws of the State of Connecticut, without reference to any choice of law doctrine.
- Arbitration. The parties agree that any claim or dispute arising from or related to this Release shall be settled by biblically based mediation and, if necessary, legally binding arbitration in accordance with the Rules of Procedure for Christian Conciliation. Judgment upon an arbitration decision may be entered in any court otherwise having jurisdiction. The parties understand that these methods shall be the sole remedy for any controversy or claim arising out of this Release and expressly waive their right to file a lawsuit in any civil court against one another for such disputes, except to enforce an arbitration decision.
- Execution. This Release may be executed electronically, and any such electronic signatures shall have the same force and effect as original signatures.
BY SIGNING, THE PARTICIPANT ACKNOWLEDGES THAT HE/SHE HAS READ AND UNDERSTOOD ALL OF THE TERMS OF THIS RELEASE AND THAT HE/SHE IS VOLUNTARILY GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE THE ORGANIZATION. THE PARTICIPANT ALSO ACKNOWLEDGES THAT PRIOR TO SIGNING THIS RELEASE HE/SHE HAD THE OPPORTUNITY TO CONSULT WITH AN ATTORNEY TO REVIEW THIS RELEASE.

If the Participant is under 18 years of age, a parent or legal guardian must also sign. I am the parent or legal guardian of the minor named above. I have the legal right to consent to and, by signing below, I hereby consent in all respects to the terms of this Release. I authorize the Organization to obtain medical treatment for such minor and release it from liability in accordance with this Release.
